Helicopter Crash in Caruaru Leaves 2 Dead and 1 Survivor, FAB Investigating Causes

A Robinson R44 helicopter, registration PR-OPS, crashed in  near Caruaru, Brazil on Monday (09 Sep 2024)  resulting in two fatalities and one survivor.

The pilot, André Luiz Gomes Ferreira, aged 44, died at the scene, while Henrique da Cunha Santos was rescued alive by locals and taken to a private hospital in the city. Henrique's health condition has not yet been reported. The third victim, whose identity has not been disclosed, also died at the scene.

According to ANAC (National Civil Aviation Agency), the helicopter's airworthiness certificate was expired, raising concerns about its fitness for flight.

The fire caused by the aircraft's fuel after the crash was brought under control within about 20 minutes by the responding fire brigade.

The Brazilian Air Force (FAB) issued a statement to Folha de Caruaru, informing that investigators from the Second Regional Service of Investigation and Prevention of Aeronautical Accidents (SERIPA II) have been activated to conduct data collection at the accident site.

This process includes assessing the damage to the aircraft and gathering relevant information to determine the causes of the crash. The goal of the investigation is to prevent similar accidents in the future.

Investigators from the Second Regional Service of Investigation and Prevention of Aeronautical Accidents (SERIPA II), a regional body of the Center for Investigation and Prevention of Aeronautical Accidents (CENIPA), based in Recife (PE), were deployed on Monday (09/09) to conduct data collection for the incident involving the aircraft registered as PR-OPS in Caruaru (PE).

During the data collection process, specific techniques are used by qualified and certified personnel. These experts are responsible for gathering and verifying data, preserving key elements, conducting an initial assessment of damages caused to or by the aircraft, and gathering other necessary information to support the investigation.

Preliminary information suggests that the aircraft arrived disassembled at Caruaru airport, where it was assembled before the flight. Additionally, the helicopter's airworthiness certificate had expired, raising concerns about its suitability for flight.
