A United Parcel Service (UPS) McDonnell Douglas MD-11(F), registered N259UP, crashed shortly after takeoff from Louisville-Muhammad Ali International Airport (SDF) in Kentucky on Tuesday, November 4, 2025, at around 17:14 local time. The cargo aircraft was operating flight UPS2967 to Honolulu when it lost control during takeoff from runway 17R.
According to initial reports and videos shared online, the number one (left) engine separated from the wing just moments after liftoff. The aircraft then rolled sharply to the left and crashed into nearby industrial buildings, resulting in a massive fire. All three crew members on board and four people on the ground were killed. At least eleven others suffered injuries.
Weather conditions at the time were clear, with light winds and good visibility, suggesting that weather was not a factor. The aircraft, built in 1991, was completely destroyed in the accident.
Preliminary analysis suggests a possible structural or mechanical failure in the left engine mount or wing structure as the likely cause of the crash. The separation of the engine during takeoff indicates a catastrophic failure that made recovery impossible. The U.S. National Transportation Safety Board (NTSB) has launched an investigation to determine the exact cause.
The Louisville airport was temporarily closed following the incident, and emergency crews worked for hours to contain the fire. UPS expressed deep sorrow for the loss of its crew and those affected on the ground.
